R4.1 Complaint received on Community Complaints Line. Omission of staff member to report to EPA in required timeframe. Discussions held between community team & environment team to improve internal notification system. EPA to monitor future compliance with this condition 2
O6.4 Designed storage capacity of Sediment Basin not restored within 5 days of cessation of rainfall event. A dedicated team engaged to ensure water treatment commences immediately after cessation of rain. Work commenced on establishing a TSS/NTU. Appropriate Action taken by licensee 1
L1.1 Small quantity of sediment affected water overtopped controls & entered Elizabeth Macarthur Creek during cleaning of new stormwater lines. Additional sandbag bund installed to the stormwater pipe at the headwall to prevent any further water release. Appropriate Action taken by licensee 1
O6.1 Earthworks commenced prior to completion of all required erosion & sediment controls. Earthworks ceased until all required erosion & sediment controls installed. Superintendent attended the Blue Book training on 3/3/2015. Appropriate Action taken by licensee 1
L4.1 Rolling/sweeping of newly lain asphalt at Tempe St continued till 6.15pm instead of 6pm. Hotmix asphalt delivery was late due to truck breakdown. Toolbox talk completed with the workforce regarding the Hours of Operation & Out of Hours Works. EPA to monitor future compliance with this condition 1
L4.2 Inaudible works past approved time of 8pm till 9.25. Contractor used generator to power lights which was contrary to approval. Subcontractor completed an investigation into non-compliance with corrective actions identified to prevent a recurrence. EPA to monitor future compliance with this condition 1
